The documents on this page are forms for various awards for scouts and adult leaders. Forms for most religious awards should be printed, filled in and submitted to Dave Pfeifer ( This e-mail address is being protected from spambots. The costs for these awards are paid for by the United Methodist Men at JCUMC through Scouting At The Creek.
One religious award which should be sent to a different leader is the John Dorman Duty to God Award. Information Flyers and Nomination Forms can be found on the John Dorman page on this website.
